Incorrectly charged for a service I'm not getting

I've been charged $4.99 per week for a subscription I haven't signed up for and a service that is not being provided. Can you please sort out a refund?

Can you please confirm how these charges appear on your account? (it will be one of the below) 

Telstra Applications Melbourne = Subscribed via Website
GOOGLE *Telstra CorpGOOGLE.COM = In App subscription via Google Play
APPLE ITUNES STORE SYDNEY = In App Subscription via iTunes

 

This will point us in the right direction to assist further.
