Foxtel Go: cannot successfully set up a Foxtel ID

Has anyone successfully set up a Foxtel ID? I have a Foxtel from Telstra account, and I have tried to set up a corresponding Foxtel ID but I keep getting the following error (see screenshot)

 

If I try to reset the password via the second link, I never receive an email with the reset link

 

I called Telstra Support late last week to ensure that my credentials (email address, DOB etc) have been correctly transferred to Foxtel, and this apparently is the case. Yet I am still at a point where I cannot get past first base.

 

Any assistance would be greatly appreciated!error received
