Why is my account stuck in provisioning status?

Our FTTP bundle is now two days past its promised connection date and we still do not have service. We have been told the account is stuck on provisioning status and two tickets have been raised to escalate the matter. Just wondering why is it "stuck" and what needs to happen for it to progress so that we can have the internet up and running asap.
