TelstraUCM.exe Application Error

When plugging in my MBB USB instead of the manager launching to connect I am getting a pop up saying telstraUCM.exe application error advising the application was unable to start correctly click OK to close the application and can't seem to launch the icon

It is also flashing one green light when plugged in
