transferring ownership or prepaid mobile

After filling out the forms to change a mobile phone from my company to me personal I was told the change of ownership could not be processed as I had not filled out the webforms.  But I wasn't told where to find these webforms and despite my searching the telstra sight and even googling to find them I have had no luck.

 

Does anyone know how to find the Webform for change of ownership of a mobile phone?

If you are transferring a service from Business Account to Consumer, you have to complete a Business  to Consumer Transfer form. You can download the webform from this link : http://www.telstra.com.au/help/download/document/change-of-ownership-b2c.pdf 

 

You can either email the form to changeofownership@telstra.com or faxed to 03 8601 2361. 

 

Make sure the necessary details are filled out and scanned copy of your 100% ID is included to avoid delays in processing your request.
