Start by giving 133933 a call and report the problem. Either way, that will get the ball rolling.

@Jupiter is correct, but you could also report the issue here: http://tel.st/rteux but be aware if you believe the cable presents a risk to public safety, the form will direct you to call a number it will provide immediately.
