TRansfer ownership of device

On my account  - I have a data sim "Go business Data Sim" which I wish to transfer to the Uniting Church in Australia - Donald.   <removed for your privacy>

I believe we could use the device for low use Internet access for the Church, for the monthly sum of $15.00.

If this is correct, what is the transfer procedure.

The device is surplus to the needs of my business.

There's a form which needs to be filled out, just google it. I don't know how to forward a link! But with the change of plans now, that plan probably isn't available anymore
