PUK is not shown in My Telstra application or website

I need the PUK for a SIM.  It is NOT accessible via My Telstra.

 

The website shows blank space under the heading PUK.

 

The application does not provide an option to view the PUK.

 

P.S. I have two services, I can get the PUK for the other one fine.  I know what I am doing.  The database is broken or missing data for this SIM.
