Last time I had a SIM replaced by a smaller SIM it was free. The old number and account details were programmed onto new blank SIM supplied by the store  free of charge at my local Telstra store.

If you want to keep your old Telstra number and plan don't buy a new prepaid pack or SIM as these come with there own number and account programed to the SIM and you will find it very difficult if not impossible to transfere your old number to the new SIM.
