Bug report - Payment extension request

Hi, 

Could find any bugreport sections to send for the website so i hope this information can be forwarded some way.

the "https://say.telstra.com.au/customer/forms/another-payment-extension" form has an issue with the JS datepicker being used, you cannot actually select a date and must manually enter a date by editing the html attribute value or changing it from read-only to non read-only.

all values are greyed out and unable to be selected in the date picker, presumably its deprecated, though console prints out a devtools failed to load sourcemap error.

Furthermore, somehow the previous extension value was assigned to 2021-27-12 causing it to invalidate all the selections.

Hey calel. 

I've flagged this with our back of house guys so we can address this and get it fixed up. I've not yet had a response, though I'll keep my ear to the ground if anything comes up. Thanks for bringing this to our attention.
